Pay for a Blue Badge, upload evidence or book an assessment
Payment is only taken when eligibiltiy is established.
You should have your application reference number ready. This starts with SR followed by 7 digits. It was given to you when you submitted your application.
All assessments are done by telephone with an Occupational Therapist. While the online booking form may display a physical venue, please disregard this, your appointment will take place over the phone.
If you are asked to book your assessment online, it will occur at the date and time you select. If we arrange the appointment for you, it will take place at the date and time allocated to you.
Make sure you provide a current contact telephone number and are available to answer the call at the scheduled time.
