Service delivery - adult social care programme

This programme helps Cheshire East Council follow the rules for Continuing Health Care (CHC) and make sure people get the right support. It aims to help residents stay independent and avoid needing extra care. The programme will improve services like Learning Disability, Supported Living, Residential care, and Shared Lives, including using technology to help. It also plans to use technology for support and make placements more cost-effective.

Health and Social Care Partnership Case Review This project has assessed how the council is discharging its responsibilities in line with Continuing Health Care (CHC) National Framework guidance and that people requiring support are managed by the appropriate organisation based on their needs. The remaining recommendations are now being implemented through business as usual.

Increase in funding from health partners to support care of residents.
Prevent, Reduce, Enable Project  This project is focused on how Cheshire East can put in measures to enable residents to maintain their independence and consequently prevent or delay users needing commissioned care, reduce the numbers of referrals into commissioned care and enable residents to live lives that do not require commissioned care for as long as is appropriate.

Improved resident quality of life including more independence for residents, better health outcomes and wellbeing, and reduced caregiver strain.

Measured by reduced hospital admissions, care home placements and participation in community-based programmes, reduction in family carer breakdown.

Embedding use of Technology Enabled Care

This project looks how the technology across the service can be used to meet resident needs when compared to traditional packages of support.

Reduction in care home placements.

Commissioning and Brokerage

This project is focused on facilitating a more cost-effective placement uptake by the implementation of a guide price policy and supporting decision rights, as well, as assessing maturity of the commissioning service.

Improved value for money from commissioned services and placements.
