Faith school forms
Some faith schools ask for a supplementary information form as part of the application process. These forms usually ask about religious practice.
You’ll often find the form on the individuals school’s website. If not, contact the school to request one.
You must send the completed form and any supporting documents directly to the school. This might include a baptism certificate or a letter from a priest confirming regular attendance by you and your child.
If you need help filling in the form, contact the school.
Schools use this information if they have more applications than places and need to follow their admissions policies to decide who gets a place.
