Managing your health online

There are many websites and apps available in Cheshire East that you can use to:

  • track and set goals to improve your health and wellbeing
  • get information about how to live well
  • manage your GP appointments and prescriptions

Health apps and websites

In Cheshire East, you may have access to use:

MyCareView is an online space that you control, powered by software PatientsKnowBest.

It contains your unique health and care information, that can be shared with your GP, hospital or anyone else you invite to see your information. It allows you to:

• See new letters, leaflets or videos shared to support your healthcare

• Communicate with anyone you have invited to see your MyCareView record

• Record your symptoms

• See and act-on any care plans that you might be on

• See upcoming appointments

• Diagnoses

• Allergies and medications

• Monitor your fitness and health by linking your account with your FitBit and other devices.

• Link your account with the NHS App if you are registered with a GP in east cheshire

If you have recently received care, or have an upcoming appointment with Mid Cheshire Hospitals NHS Foundation Trust or East Cheshire NHS Trust, you can now download NHS App and link it with your MyCareView account. Either hospital may also invite you directly as part of the care they deliver, but as a patient you can now choose when to register by using NHS App, you do not have to wait for an invitation.

Patient Access

A secure online and mobile application offering access to a range of GP Services as well as access to your health record. It allows you to:

  • order repeat prescriptions
  • book, view and cancel appointments with your GP, nurse or clinician
  • access your health records, including test results and details of immunisation and allergies
  • have secure messaging with your practice
  • securely share your information with healthcare professionals of your choosing, without the need to contact your practice

Evergreen Life

An online and mobile service which supports all the main GP systems in England. Requesting access to online patient services at your GP can enable you to:

  • view and download your medical record
  • book and cancel appointments
  • add appointments into your calendar
  • order repeat prescriptions
  • send a secure message to your GP 
  • read the latest news and updates from your GP
  • view your personal health record and take control of your health

Evergreen Life also provides a number of other useful benefits, including:

  • medication reminders
  • ability to store documents
  • a health and fitness monitor which tracks blood pressure, body fat percentage, blood glucose and more
  • trusted access to who you want at the level that you want
  • a timeline providing a complete view of your health in historical order.
  • an Evergreen Life DNA testing kit is available to order from within the app.

eConsult

eConsult provides an online consultation portal.  It gives patients the ability to:

  • eConsult with their GP
  • self-check symptoms
  • receive instant self-help advice
  • be signposted to NHS 111, pharmacies and other health services

National online services

Catch 0-5

Catch 0-5 is an NHS Health app for parents and carers of children aged 0-5. It offers the following benefits:

  • personalised advice – when you create a profile for each of your children the app will list health articles relevant to your child’s age
  • health information and advice drawn mainly from NHS Choices
  • updates with reminders about key health dates, such as childhood immunisations and seasonal advice. This also includes health information from your area such as bank holiday pharmacy opening times or urgent health news
  • maps of available local health and social care services within your area. This includes your nearest GP surgery, dentist and pharmacies
  • emergency information about symptoms and situations which require urgent medical help, such as allergic reactions or head injuries. In the event of an emergency this feature includes 'Call 999' and 'Call 111' buttons.

NHS App

The NHS App is for patients aged 13 or over to access a range of healthcare services online and will be integrated with the four IT systems used by general practice. Features of the App:

  • check their symptoms using the health A-Z on the NHS website
  • find out what to do when they need help urgently using NHS 111 online
  • book and manage appointments at their GP surgery
  • order their repeat prescriptions
  • securely view their GP medical record
  • register as an organ donor
  • choose whether the NHS uses their data for research and planning

The app is being rolled out gradually across England, following a successful pilot and is the first major platform to use the new NHS Login (for verifying a citizen’s identity).
