Lifestyle Prescription - Your Lifestyle (PDF, 4.6MB) resource is available to help with making life changes.
If you have been diagnosed with a long term condition, Lifestyle Prescription - Your Long Term Condition (PDF, 4.5MB) resource is available to support you.
Lifestyle Prescription - Your Treatment (PDF, 4.5MB) can support you when starting a new treatment for a long term condition.
For professionals
There is an array of local and national support available, some of which we have included in the resource. We worked alongside pharmacists, social prescribers, doctors and local residents, with input from the NHS, council and Voluntary, Community, Faith and Social Enterprises colleagues (VCFSE) to develop the resource. We also piloted it back in January 2023.
For Cheshire East Council staff, there is the Healthy Lifestyles E-Learning on the Learning Lounge.
