What is our Care4CE pathway?

At Cheshire East, we believe everyone working in care can make a real difference and deserves the very best start and ongoing support.

Working closely with our Care4CE team, we have launched the Cheshire East Care4CE pathway. This pathway offers in-depth training and supports professional development at every stage. It gives you the experience, skills and tools you need to succeed and become the best professional you can be.

If you are at the start of your care work career journey when you join Cheshire East, you will have a full induction into your role by a senior member of staff.  You will complete the online induction training courses and will shadow other care workers until you are confident to work on your own. 2 months into your post you are required to complete the Care Certificate, a senior member of staff will support you with this.

For those who join Care4CE  with prior support work or care experience we can provide you with clear progression and development routes and opportunities. We offer a variety of learning and professional development, where our commitment to collaborative and inclusive working ensures you have the assistance and support you need to perform your job to the best of your ability each day and to make a real difference to those you encounter.

What sets us apart is that we believe you should be in full control of your development and that there shouldn’t be just one route or direction for you to go in. We offer more than one pathway to make sure you can flourish and develop into a route that suits you  and your future aspirations best.

A Community Support Worker has a varied and fulfilling role within Cheshire East working with our service users to help attend to their daily needs whilst maintaining their independence. This can range in nature from physical tasks, such as dressing, washing and feeding through to social tasks such as helping them to talk with individuals and helping them to maintain contact with their family.

A Community Support Worker is vital for creating an environment which allows individuals to meet their full potential whilst ensuring that support plans are followed and that accurate records are kept to ensure we are providing the best level of service that we can.

A reablement worker attends to the personal needs of customers, encouraging them where possible to undertake physical tasks for themselves, including all aspects of personal care and administering prescribed medication.

They contribute to the development of support plans and risk assessments, interpreting and implementing them to ensure agreed outcomes are met.

They undertake tasks as required on behalf of Health and Social Care professionals following instruction and training and maintain all relevant records to ensure clerical and management information is available for decision making.

A Senior Support Worker performs an import role within Cheshire East as a key worker to a number of our service users, working in partnership with them to identify and co-ordinate their individual support plans. They contribute to the process of assessment, implementation, recording and review of individual support plans to service specification standards to ensure our service users receive the highest level of care.

As with all our Community Support Worker’s they create an environment which allows individuals to meet their full potential whilst ensuring that support plans are followed and that accurate records are kept to ensure we are providing the best level of service that we can.

Service Co-Ordinators at Cheshire East co-ordinate resources and the work of our care team to incoming referrals to ensure we achieve the required outcomes for our service users. They have to ensure that they prioritise referrals with special difficulty, sensitivity or high risk working closely with our Team Leaders and Senior Care Assistants, to ensure resources are allocated according to need.

They act as a pivotal point of contact by taking telephone calls from service users and their relatives regarding service delivery and non-delivery and any cancellations to services.

A Social Care Assessor manages the recruitment process of family-based carers and matches the skills and experience of our family-based cares to our service user’s needs, ensuring the right placement is made to provide the highest level of care possible.

They monitor service users and placements, providing feedback to the relevant teams and individuals to make sure our services users receive care to meet their needs.

A Senior Supervisory Support Worker provides day to day and group supervision of our Community Support Workers to ensure that customers support programmes are effectively implemented, and that tasks are completed to the required standard. They are responsible for delivering inductions and training for our Community Support Workers and ensuring that our teams have the required knowledge and equipment to perform their job to the highest level in a safe and compliant manner.

They ensure individual plans are outcome focused; that they are implemented and reviewed in a timely way, and any changes are communicated to the relevant people to ensure we continue to provide the highest levels of care to our service users.

An Outcomes Co-Ordinator manages and oversees the delivery of social care services in line with service aims and objectives, ensuring that regulatory and statutory requirements are met, and, where required as Registered Manager ensure adherence to CQC standards in order to deliver high quality services, and ensure that key performance targets are delivered and reported on.

They help to recruit, retain and manage team members, deal with performance issues, including capacity and conduct issues through the application of related HR Policies and Procedures and identify and facilitate learning and development needs to ensure that staff have all the necessary skills and knowledge to provide the highest levels of care for our service users.

A Resource Manager manages and has responsibility for the delivery of social care services in line with service aims and objectives, ensuring that regulatory and statutory requirements are met, and, where required as Registered Manager ensure adherence to CQC standards to deliver high quality services.

They are responsible for budgetary matters and budget management related to the resource, ensure effective financial control and for day-to-day decision-making in respect of the building (fire precaution, upkeep of maintenance etc.)

They are responsible for recruiting, retaining and personnel management of team members and personnel, whilst identifying and facilitating learning and development needs to ensure that staff have the necessary skills, aptitudes and competences to deliver flexible and responsive services.
