Extra Help forms and support for professionals
Extra Help forms
The Lead Practitioner should keep completed Extra Help forms, including any direct work completed with children and young people. You do not need to send Extra Help assessments or forms to Partnership Family Help Officers for early help support. If needs increase and Family Help support is being considered, the Extra Help Assessment and Plan may be requested.
Continuum of Need
Refer to the Multi-Agency Continuum of Need Guidance on the Cheshire East Safeguarding Children’s Partnership website:
Consent and information sharing
When discussing consent and information sharing with children, young people and their families, explain how their information will be stored.
Professionals completing Extra Help forms should keep the original signed copies securely, in line with their own agency procedures. If you are asked to send it to us electronically, please write 'signed copy on case file' on the signature line.
If you do not have access to secure email, use Egress to send information securely.
Additional needs
If a child or young person has additional needs, you may find the following specialist resources helpful:
Child or adolescent violence to parents
Toolkit for professionals
This toolkit supports professionals working with families where there are concerns about child or adolescent violence towards parents. It includes guidance, session plans and worksheets for use with parents and children.
The toolkit is designed to help families develop communication and conflict-management skills that they can continue to use independently.
