What is Reablement?
Reablement is a new service based on national government policy.
It aims to give you greater control of your life and the support
you receive. Cheshire East Council Adult Social Care offer a
Reablement service to help you at a time when you need extra
support to help you to remain in your own home and be as
independent as possible.
How does Reablement work?
Reablement is only available to customers over 18 years of age
who have substantial or critical needs. One of our social care
assessors will visit you to talk about your needs, and if
appropriate will refer you to the Reablement service.
Our Reablement worker will visit you and talk to you about what
you want to achieve, and how this could happen. Our workers will
support you to plan your own goals, and how these can be achieved.
Over a short period of time (a maximum of six weeks) our Reablement
Workers will visit you and provide a range of support, for example
this could be for personal care, shopping, or picking up threads of
your life that are important to you.
Our Reablement workers will also work with you to help learn new
skills or relearn skills which have become difficult.
What will it cost?
The Reablement service is free, however you might be charged for
some other things such as meals and transport. To decide whether or
not you will need to pay towards any services that you have after
the free Reablement period, you will undergo a financial
assessment.
