Knutsford Supported Living Network
Knutsford Supported Living Network offers support to
people with learning disabilities living in their own
homes. We will support you with all elements of your
daily living skills, allowing and enabling you to maximise your
independence and promote wellbeing. The level of support we offer
to each individual will vary depending on their current needs.
The Network Service operates 24 hours a day for 365 days a year,
and supports 36 people. Over 60 staff are employed by the
service.

Heather Brae Supported Living Network
Heather Brae
is a supported living network in Congleton which supports people to
live independently. Services are offered 24 hours a
day, everyday, supporting you according to your
individual needs.
We support people with with a wide range of abilities and
complex needs. These will include people with learning
disabilities, multiple needs, mental health issues and physical
disabilities.

Wilmslow Supported Living Network
Wilmslow Supported Living Network is a supported living
service enabling adults with learning disabilities to live as
tenants in their own homes.
We will support you in ways that will enable you to live a full
and valued life in your community.
We will support you to make choices and decisions, to develop
and achieve your potential and to maintain your contacts and
interests.
Some of the opportunities include learning and developing new
skills, working towards agreed goals and achieving as much
independence as possible. We will also support and enable you to
access local leisure and social activities.

Macclesfield Supported Living Network
Macclesfield
Supported Living Network offers accommodation for adults with
learning disabilities. We offer support to people with individual
tenancies and within shared accomodation. Each property has
comfortable lounge/living spaces as well as private space for each
tenant. It is within 20 minutes walking distance of the town
centre. Local public transport is nearby and there are lots of
local taxi companies.
Macclesfield Supported Living Network is designed to give
you your own tenancy in a home you can make your own. You
will be supported to be independent, to do things
that you enjoy, and to live a healthy life. Staff are
available to help you plan and live the life you want,
both within your own home and within the wider community.
Fully trained staff and on-call cover is available 24 hours per
day, all year round. You are supported to lead a
valued life, make choices and decisions for yourself, develop and
achieve your potential and to maintain your contacts and
interests in the wider community. We have close connections
with local day services and other community groups which you
can attend if you choose.

Crewe and Nantwich Supported Living Network
The Network provides supported living in the areas of Crewe
and Nantwich and enables you to live on your own or with
others.
We can support you to develop skills in household
tasks, budgeting, personal care and weekly planning.
We can offer information and support so that you can
attend work, college or day time activities, including spiritual,
leisure and social interests. We can support you to make
informed choices about your health and well being.
We ensure that your service is monitored and reviewed to make
sure it continues to meet your needs. We can help you to
discuss your accommodation with your housing provider.

Congleton Supported Living Network
The Network Service offers support to adults with a
learning disability to live within their own home.
We can enable you to maintain and develop independence.
We can offer the following support based on your needs,
choices and preferences to enable you to reach positive
outcomes.
What we can offer:
- Personal care
- The administration of medication
- Housing related support
- Managing your tenancy
- Liaison with your landlord re: repairs and maintenance
- Independence around the home
- Cultural specific support
- Safety and security of the home
- Support during the night
- Support you to access health care when you need it
- Work in partnership with you, your family and others to ensure
we have assessed, planned, provided and reviewed your service
- Provide support for you to manage your financial needs i.e.
paying your rent, banking
- Make referrals for you to access specialist services or sign
post you to the appropriate services
- Support you to access transport i.e. lease vehicles
- Enable you to access your local community and local
services
- Adult protection

Domiciliary Service
Care4CE's
Domiciliary Service can work with you in your own home for a
short period of time.
We will discuss with you why you need help and what is causing
you difficulties.
Our team of Re-ablement workers will help you plan how to solve
these difficulties, and will offer personal support for up to
six weeks to help you regain or relearn daily living skills.

Shared Lives, Congleton
Shared Lives offers a personalised
service to vulnerable adults, providing care within a family
setting. This may be in the home of an approved Carer, in that of
the person needing care or in the community.
The Shared Lives Team matches carers
to customers and is responsible for the recruitment, approval,
supervision and training of carers.
There are three main types of support
offered:
- Sessional support – where building based day
services are not appropriate.
- Respite – a flexible & responsive
service, including a sitting service & carers breaks.
- Long term (intermediate) support.
The team can also assist young adults, who
have been in foster placements, to move into the Shared Lives
service.

We are a small team of experienced mental health workers who
work in partnership with Community Mental Health Teams and the
Improving Access to Psychological Therapies Service and Drug and
Alcohol Teams.
We can provide you with up to 6 weeks free support following a
Re-ablement model. This may include advice to access appropriate
support services, debt management, housing issues, education,
employment, leisure activities etc.
The support will focus on coping techniques and a self-help
approach, promoting social inclusion, building self-esteem and goal
setting.
