Hollins View Community Support Centre, Macclesfield

Hollins View Community Support Centre Hollins View Community Support Centre offers a range of flexible residential, Intermediate Care and Re-ablement short breaks and day services. These offer support in a community setting to enable you to remain in your own home. Our caring, well-trained staff will do their best to help you have an enjoyable stay with us, tailoring your care and encouraging activities suitable to your needs and requests. The staff are responsible for supporting your wellbeing and personal care during your stay. They will offer dependable and discreet support with tasks and will assist you towards achieving your overall aims. Staff are available 24 hours a day and will be glad to support you at any time. Whilst accessing the services here, you should feel that you have been able to exercise control and choice and that you have been treated with dignity and respect.

Lincoln House, Crewe

Lincoln HouseLincoln House Community Support Centre offers a range of flexible short breaks, Re-ablement and day services and specialised services for people living with Dementia. In June 2010 Lincoln House reopened after the completion of a £1 million modernisation and extension project.  Lincoln House can now provide short breaks for up to 44 people in modern and well designed accomodation. The Santune Suite can also provide short breaks and day services for up to 12 people living with Dementia.  We offer support in a community setting to enable you to remain in your own home. Our caring, well-trained staff will work in a person centred way to enable you to have an enjoyable stay with us and tailor your care, support and activities around your your needs and requests. The staff are responsible for supporting your wellbeing and personal care during your stay. They will offer dependable and discreet support with tasks and will assist you towards achieving your overall aims. Staff are available 24 hours a day and will be glad to support you at any time. Whilst accessing the services here, you should feel that you have been able to exercise control and choice and that you have been treated with dignity and respect.

Mountview Community Support Centre, Congleton

Mountview Community Support CentreMountview is a 35 bedroomed care home providing respite, Re-ablement and day services for older people. At Mountview there is a relaxed, friendly atmosphere, home cooked food and caring, well-trained staff will do their best to help you have an enjoyable stay. They are responsible for your well being and personal care and will offer dependable and discreet support 24 hours a day. Staff are available to assist you and your family towards achieving your overall aims and whilst here you should feel that you have been able to exercise control and choice and that you have been treated with dignity and respect.

Queens Drive, Nantwich 

Queens Drive Respite HomeQueens Drive offers short breaks to adults with a learning disability. Queens Drive is a large detached house, near to Nantwich town centre.

There are six bedrooms including one room that is used for emergency placement if necessary. We offer short breaks in a homely environment and support you to do the things you enjoy.

How long you stay will depend on the assessment carried out by the SMART Team based at Delamere House.

Warwick Mews, Macclesfield

Warwick Mews, MacclesfieldWarwick Mews short breaks service in Macclesfield provides respite for adults in Cheshire East.

We can offer you a short break in a friendly and supported environment.  We will support you to be independent and do the things that you enjoy.  How long you stay will depend on an individual assessment and could range from just one night up to twelve weeks.
