Transition to Adult
Life Information
Parent Partnership Service Cheshire East offers information,
advice and support on education matters for parents and carers
whose children have special educational needs (SEN). This
information is for parents and carers who have a child in Year 9
with a Statement of Special Educational Need. We hope
this transition to adult life information pack will give you a
clearer idea of what to expect when you are invited by the school
to your child’s review this year.
In Year 9, the Annual Review meeting to discuss your child’s
Statement is called a Transition Review. This time, as well
as talking about progress over the past year and targets for the
next, you and your child will also begin to look forward to the
future years until he or she leaves school. It is an
opportunity to begin to make plans. Your views and those of
your child will be a very important part of the discussions.
After the review your child’s school will send you a written
Transition Plan, outlining the ideas you talked about at the
meeting. The Connexions Service will also work with your
child in school, as the plan is reviewed each year.
We know that children’s needs cover a wide range. You may
find some of this information is not relevant to your child, but we
hope there will be something of interest to everyone.

- The Connexions website has a wide

Transition Information Network (
TIN
)
- The Transition Information Network support disabled young
people in their transition to adult life. They have
information for parents, carers, young people and
professionals. They also produce "My Future Choices" magazine
that can be downloaded from the Transition Information
Network website.
Family Lives
- The Family Lives
website has a section dedicated to teenagers that covers a
range of topics such as boundaries, drugs, sex, health and
school.
Contact a Family
- Contact a Family produce a leaflet "Preparing for adult life"
which can be downloaded for free from the Contact
a Family website.
